Filling Out the HIC Form & HIC Client Form

The HIC Form is the heart of the annual Housing Inventory Count which is submitted to HUD on an annual basis. The form contains information about a project's bed inventory, as well as other information about how the project operates. A draft of this form will be completed by the HMIS team for each housing project for all agencies participating in the HIC/Sheltered PIT. Agencies will review these forms, and update the data as necessary. A new form only needs to be completed for a project if a form was not created by 211OC.


Agencies not participating in HMIS must also submit the HIC Client Form for any permanent housing projects to ensure the data reported is accurate.

211OC will create a ticket with links to HIC Forms for each housing project that is active at the agency. Review this data and make revisions so the data is accurate as of the night of the Housing Inventory Count.

  • The form includes additional details regarding data collection for each field. 
  • Fields will appear or disappear from the form depending on how certain fields are answered. This ensures the user is only answering questions they need to answer.
  • To save updates to the form without submitting, click on the Save button at the bottom of the screen.
  • To submit the final form to 211OC, enter an explanation for any revisions to the form, and click on the Submit to the OC HMIS Help Desk button.
  • Once the final form has been submitted, please notify the HMIS Help Desk.



HIC Client Form Instructions

PSH, OPH, and RRH projects that do not participate in HMIS are required to provide client-level information so that bed inventory information provided on the HIC Form can be validated against client data. Blank HIC Client Forms can be found in the 2023 HIC and Sheltered PIT knowledge base article. In order to streamline the data review and data entry process, information about each column on the form can be found right inside the HIC Client Form. Hover over any cell colored light yellow to see more information about the column:



Each non-HMIS participating PSH, OPH, and RRH project should have its own separate HIC Client Form, filled in with the required information about each client who was active in the project and in permanent housing on the night of the HIC. Clients are considered active if they do not have an exit date for the project, or the exit date is after the night of the HIC/Sheltered PIT.


When you have completed your HIC Client Forms, please upload your completed forms to your agency's Dropbox folder.
