Bed Reservation Custom Exit Questions: Response Explanations

Background 

When completing a program exit for any of the Emergency Shelters available in the Bed Reservation System (BRS), users will find two custom questions that capture the following:

 

  • If the client’s exit was voluntary or involuntary.
  • If the client had a current behavioral contract.
  • Client’s eligibility to return and how soon they can return.

 

Responses to the OC Bed Reservation Custom Questions may impact a client’s eligibility for future shelter placement.

 

Users are encouraged to review the available response explanations show below to ensure the selected responses are accurate. 

 

BRS Custom Exit Questions Response Explanations

Question 1: Reason for Exit

Response Options Response Explanation
Successful Placement

Select if...

 

Client was placed in another interim housing program (e.g. recuperative care or motel stay, Transitional Housing Program).

 

 

Client moved into Permanent Housing.

 

 

Client was successfully diverted from the Coordinated Entry System to a permanent housing situation (e.g. staying with friends or family on a permanent basis).

 

Voluntary Departure/Self-Exit

Select if...

 

The client left on their own; either planned exit or AWOL.

 

Institutional 

Select if...

 

Client was hospitalized (includes mental health hospitalization).

 

Client has been incarcerated. 

 

Violation of Program Expectations - Contraband

Select if...

 

The client has been asked to leave due to possession of contraband prohibited on-site.

 

Violation of Program Expectations - 

Behavior

Select if...

 

The client’s behavior violated the behavioral expectations agreed upon at program entry. 

 

Violation of Program Expectations -

Overdose on Site

Select if...

 

The client experienced an overdose on the shelter premises.

 

Violation of Behavioral Contract

Select if...

 

The client had a Behavioral Contract in-place and was asked to leave for violating the contract agreements. 

 

Violation of Behavioral Contact...Explanation

 

This field will appear when the reason for exit response, ‘Violation of Behavioral Contract,’ is selected.


The user must enter specific information about the violation that occurred.

 

Deceased

Select if...

 

The client has died.

Other

Select if...

 

The reason for the client’s exit is not captured by any of the response options above.

 

Reason for Exit if Other

 

This field will appear when the reason for the exit response selected is ‘Other.’

 

The user must describe the circumstances of  the client’s exit. Users are encouraged to review the previous response option explanations before selecting, ’Other.’
 

 

 

Question 2: Eligible to Return

Response Options Explanation
30, 60, 90 Days

 

Select the number of days needed to pass before the client can re-enroll in the shelter program.

 

Eligible to Return after 90-Days with Behavioral Contract

Select if...

 

The client may be re-admitted after 90-days with a signed Behavioral Contract. 

 

Upon Availability

Select if... 

 

If the shelter prefers not to provide an estimated return date.

 

No Return

Select if...

 

If the shelter provider will not allow this client to return. 

 

Eligible to Return, Details

For this field... 

 

The user must enter any important information or conditions for a client's future referral to that shelter site.
